MLSGO Student Matching Form

Welcome to the MLSGO Student Matching Form. If you've already completed the matching form, but would like to change your matching preferences enter your email address below and we will retrieve your most recent matching preferences. If this is your first time filling out the matching form, enter the email address you used when filling out your application form. The Medical Laboratory Science Group of Oklahoma (MLSGO) supports a matching program for placing their applicants. It is similar to the system used by the National Intern and Resident Matching Program for medical students. AFTER applying and interviewing with individual hospital programs, the applicant will rank each hospital in order of preference and submit his/her preferences using the form below. The hospitals will also submit a list of candidates for their Medical Laboratory Science Class in order of preference. Each hospital and applicant list will then be matched on the last Friday in January. Form submissions will only be viewed on the day of the match. The applicant will be placed at his or her most preferred hospital which has listed that applicant on their acceptance list. In this system, the student is given priority.
  1. Applicants must submit their matching forms by January 20th of each year. If applicants wish to change their original decisions, resubmit the matching form below. On the day of the match we will use the most recent submission completed by each applicant.
  2. An applicant must rank, in order of preference, only those programs that he or she is willing to accept. No program has to accept any applicant they do not want, nor does any applicant have to accept any program they do not wish to attend.
  3. Applicants will be notified of their acceptance into a program by the hospital program director.
  4. In order to confirm acceptance into a program, the applicant must submit a written letter of acceptance to that particular program by March 1st. Matched applicants will submit a $50 refundable deposit through PayPal to MLSGO in order to secure a position in that program. Deposits will be refunded two weeks after the program start date. Applicants who do not fulfill the acceptance commitment will not receive this refund, per discretion of the program director.
  5. Applicants not matched in the process will receive notification of their status from the Matching Chairman no later than February 7th. If notification is not received, write to MLSGO and mail to the address below.
